Responsibilities: The LTSS Program Manager provides oversight of the LTSS CP Care Coordination program. The role provides direction to LTSS Care Coordinators and Supervisors across the Member Organizations in the provision of LTSS care planning and coordination for MassHealth Members with complex healthcare needs who are enrolled in an Accountable Care Organization (ACO)plan. The role also serves as a Primary Point of Contact with our 11 ACO partners, collaborating with both Central ACO teams and Practice level POCs to promote timely communication and problem solving, and a high degree of integrated care. The role supports Member Organizations in addressing Social Determinants of Health, maintaining robust directory of resources, linkages, and partnerships. And the role partners with the Executive Director in ensuring that each Member Organization is meeting quality performance metrics and Enrollee satisfaction.
Qualifications: Ability to function as an effective change agent and to be flexible, open and responsive to ongoing industry changes; Ability to function under pressure in fast paced start up environment; Knowledge of LTSS resources, values, principles and techniques; Knowledge of health promotion and clinical care coordination techniques, including motivational interviewing; Knowledge of formal and informal assessment practices; Ability to triage/balance competing priorities; Ability to make independent judgments and decisions. Minimum of 3-5 years supervisory experience required. Case management experienced required. Effective skills in managing, teaching, and negotiating, and in collaborating with multidisciplinary teams and clients/families. Experience developing LTSS care plans. Strong organizational and time management skills. Excellent written and oral communication skills. Effective relationship, management and team building skills. Preference given to bi-lingual/bi-cultural applicants and applicants with lived experience. Master’s Degree required; License (LPHA) to practice as “Licensed Practitioner of the Healing Arts” in MA; MA license to practice independently (LICSW, LMHC); or Registered Nurse (RN).
