Schedule: Generally Monday-Friday 8am-4pm, but one late day per week (12pm-8pm), and weekend coverage (Sat & Sun 8am-4pm) on a rotating basis every six weeks

Responsibilities: At intake and at least annually, assess each individual’s employment and education needs and develop a plan based on each individual client’s circumstances. Assist in all phases of job development. Provide advocacy, job coaching, and educational support for individuals served by the PACT team, including coaching individuals served to complete the tasks related to obtaining employment and education, such as assisting individuals to create resumes and cover letters, strengthen interview skills, complete applications and forms, and follow up on applications and interviews. Establish and maintain relationships with each client’s manager, remain up to date on each individual’s professional development, and work to quickly resolve work-performance issues that could impact the individual’s employment. Create and maintain employment and education information in each client’s record in the electronic database. Provide benefits counseling to individuals served by the team. Act as a specialized resource for the entire team by creating and maintaining a collection of education and employment resources, by staying on top of the latest changes in employment and education rules and regulations, by keeping up-to-date on changes in resource availability, and by providing consultation to team members regarding employment and education processes and procedures. Provide case management for an assigned group of clients, including carrying out a comprehensive assessment of needs and assuming primary responsibility for developing, writing, implementing, evaluating, and revising all treatment goals and plans in conjunction with the ITT. Provide treatment and rehabilitation services to all persons served by the team by providing skills training and support in all areas of functioning including: symptom management, coping skills, substance abuse, vocational and educational, housing, activities of daily living, benefits counseling, money management, public transportation, appointment attendance, social and recreational, etc. Perform shift management as assigned according to program policies and procedures. Provide crisis assessment, prevention, and needed intervention during the work day and while on-call on nights and weekends as scheduled. Complete all required documentation including ACAs, IAPs, service notes, incident reports, outcomes reporting, monthly employment reporting, etc. Complete other duties and tasks as assigned by supervisor. Qualifications: A Bachelor’s Degree in Rehabilitation, Psychology, Social Work, or a related area. At least two years of experience working with individuals diagnosed with severe and persistent mental health issues or similar human services needs. Experience assisting others in education and employment pursuits preferred. Professional writing and oral communications skills. Demonstrated commitment to the belief that all individuals have the capacity to be competitively employed, should they so desire. High degree of personal organization, attention to detail, and time management skills. Proficiency in word processing and other computer functions using Windows, Word, and Google Email and Apps. Ability to climb stairs and carry up to 40 pounds. Valid driver's license and use of one’s personal vehicle.
